Practice Philosophy


The philosophy of our practice is to provide optimal periodontal health care and implant surgery to our patients through a team effort. We strive to render treatment on an individual, caring, and personalized basis in a relaxed and professional atmosphere. A unified effort by our periodontal healthcare team will enable us to achieve our goal providing superior care to our patients. We strive to learn from our current experiences and continue to integrate innovations necessary for our improvement and growth.

Our Methods

First we will get acquainted with you and your past dental and medical history. If you need X-rays, they will be taken. Then we will do a complete examination of your mouth, including measuring of periodontal pockets (in millimeters), and examination of your X-rays. We may need to take impressions of your teeth for further study. These procedures help us determine how healthy your mouth is, or to what extent it has been damaged. This will usually be accomplished on the first appointment.

You will be advised at your second appointment of our findings. Using the diagnostic information, we will work out details and the sequence of your special treatment. We will then be able to answer any questions you may have concerning your treatment, the time it will take, and what it will cost.

Billing

Fees are based on the time spent with you and the dental procedure rendered. If you have any questions regarding fees, and/or alternate payment schedules, the front desk coordinator will be glad to discuss them with you.
We will assist you in filling out the claims for payment by your insurance company, providing the forms are uniform and in accordance with the standards of the Health Insurance Association of America. We shall report only what is to be performed and when it was done. It will be a true and accurate report from our office records.

The amount you receive from your insurance company depends solely on the terms of your contract with that company. Health insurance is a contract between the insurer and the insured. Therefore, the patient is responsible for full payment of all charges not covered by their insurance policy.

Appointments & Cancellations

If you cannot keep an appointment, reasonable notice (at least 48 hours) should be given to our office to cancel your appointment. This allows us to be able to give this time to another patient.

Patient Forms

The Mark Forrest, D.M.D. provides our patients with office forms for your convenience. We recommend you print and fill these forms prior to your appointment to save time.
